Не удалось получить переменные get для сброса пароля

Я создал веб-сайт и сейчас пытаюсь создать способ, позволяющий пользователям сбрасывать пароль, когда они забывают. Я успешно создал URL-адрес для сброса пароля, но каждый раз, когда я нажимаю на него, я получаю «Неверный запрос!» Уведомление скорее берет меня на страницу index.php. Буду признателен, если кто-нибудь сможет помочь. Мои коды ниже.
